Very topical given the findings in today's newspapers! This is an excellent example of a survey with one or two qualitative questions added. You can indeed put 'free text' questions in a survey but you won't get the rich responses that you would get in semi-structured interviews which are typically tape recorded and can give you pages and pages of qualitative data.
