Students are expected to disclose all criminal punishments and investigations to the School immediately. The School additionally requires you to submit a declaration and disclosure questionnaire on an annual basis, even if you have nothing to declare.
Guidance on what you should declare can be found here. The questionnaire should resemble the questions you are asked by the General Medical Council in your application for provisional registration. If you answer yes to the question regarding health then the Student Support Office will be in contact with relevant information and you can meet with them if you wish.
You are reminded that if you declare something which could reasonably have been declared in an earlier annual declaration and disclosure process (such as when you were asked in Year 2) you will need to explain why you did not. This may lead to proceedings being initiated under the Professional Capability & Fitness to Practise Policy.
